Office Hours and Policies

Office Hours
Phones will be answered from 7:30am to 5pm weekdays. Providers are generally available for appointments at the follow times:

Monday 8:30am – 8:00pm
Tuesday 7:30am – 8:00 pm
Wednesday 7:30am – 3:00pm
Thursday 7:30am – 4:30pm
Friday 7:30am – 4:00pm

Please note that occasionally a provider will have a conflict and will not be able to work late. In that case the office may close earlier. Please call before coming!

Appointment Policy
Although we are flexible and will try to accommodate the needs of our patients, we prefer that all patients be seen by appointment. We open at 7:30 am weekdays and close between 5:00 and 8:00 pm depending on the day.

Missed Appointments
If it is necessary to cancel or change an appointment we require one business day's notice. This allows us the opportunity to reschedule someone else in your timeslot. If cancellation occurs less than one business day prior to the appointment there will be a minimum charge of $35.00 for a missed appointment, and a minimum charge of $60.00 for routine physicals. In the event you are late for an appointment, we will make every effort to see you, but we may not be able to complete your full treatment as planned for that day. If you are late and have missed more than half of your appointment time, you may need to reschedule and a missed appointment fee will be incurred.

After Hours Coverage
When the office is closed, Dr. Moghissi carries her cell phone to handle emergencies. When she is on vacation, or on weekends, another local family practitioner may be covering for her. If you have an emergency that requires a doctor's advice, please call the office to get the number for the doctor on call.

Communication with the Doctor
Patients are welcome to communicate and ask questions of the doctor via email. If she feels that the problem is too complicated to take care of via email, she will ask you to come in for an office visit. Dr. Moghissi does not check her email on weekends, or when she is out of the office. Please do not email with urgent questions, it is better to call.

Results
Dr. Moghissi notifies her patients of all test results, even if normal. She often sends those results via email, (or we can call with the results). If you have abnormal test results you will be asked to come back in for an appointment to discuss your results, at which time we will review your treatment plan and issue any orders required for further treatment and additional testing. No abnormal results are discussed over the phone or by email.

Prescriptions
Our providers will give you enough medication until you are due for a follow up appointment. If you are running low on a prescription, please call our office and set up an appointment. If for any reason your prescription for medication, referrals, mammograms, x-rays, blood work, etc., needs to be re-written, there will be a $10 charge for a replacement to be called in, faxed or mailed to you. As an alternative, you may make an appointment to see one of our providers, at which time they will rewrite your prescription.

Payment and Insurance
We participate with most insurance plans, Click here for a list of insurers or call us with your specific questions. Please bring insurance information (including your insurance card) to each visit. We accept payment in the form of cash, check, MC, VISA or Discover. Payment for the office visit and/or co-pay and any previous balances will be collected at the time of service. In the event an account is not paid in full within 60 days, it becomes delinquent and it is the responsibility of the patient to pay a $25.00 late/collections fee. If a check is declined or returned, there will be a $50.00 non-refundable fee.
